RESUMO

Resumo As malformações vasculares são anomalias que podem acometer veias, vasos linfáticos e artérias de forma isolada ou mista. Quando se apresentam de forma mista, com componentes venosos e linfáticos, são denominadas malformação venolinfática ou linfático-venosa, de acordo com sua constituição predominante. Embora seja um distúrbio benigno de bom prognóstico, é localmente invasivo, podendo levar a deformidade e havendo, ainda, a propensão de recorrência local. O presente artigo traz um caso de malformação venolinfática com localização incomum em borda lateral de língua, abordando-se a conduta clínica e o referencial teórico vigente.


Abstract Vascular malformations are vascular anomalies that can affect veins, lymphatic vessels, and/or arteries in isolated or mixed form. When they present in the mixed form with venous and lymphatic involvement, they are called venolymphatic or lymphatic-venous malformations, depending on their predominant component. Although these are benign disorders with good prognosis, they are locally invasive and may lead to deformity, while there is also a propensity for local recurrence. This article presents a case of venolymphatic malformation with unusual localization on the lateral border of the tongue, addressing the clinical conduct and the current theoretical framework.

ABSTRACT Objective: To evaluate and systematize tongue color-related manifestations among patients with PCR-confirmed COVID-19 infection. Material and Methods: This retrospective study included analysis of tongue images obtained from patients with PCR-confirmed COVID-19 infection. Evaluation of coronavirus disease severity (mild, moderate, severe, critical) was provided, considering clinical symptomatology and results of laboratorial and instrumental diagnostic methods. Each picture was analyzed considering the parameters of color of the tongue and color of the tongue plaque by two dental specialists. Cochran-Armitage test for trend was used to evaluate associations between the tongue color and tongue plaque color, and coronavirus disease severity. Results: The most prevalent tongue colors were pale pink, red and dark red (burgundy color). A total of 64.29% of patients with mild disease demonstrated pale pink color of the tongue. Patients with moderate coronavirus disease were characterized with the adverse trend: 62.35% of them presented with red-colored tongue, while in 37.64% of cases, the tongue was pale pink. Severe COVID-19 patients, almost in 90% of the cases, had either red or burgundy color of the tongue. Conclusion: SARS-COV-2 infection is not manifested by tongue-targeted or tongue-specific signs and features; however, coronavirus disease itself provokes changes within the tongue color and tongue plaque color similar to those registered during other internal pathologies.

Objetivo: O objetivo deste estudo foi realizar uma revisão sistemática da literatura sobre a associação entre o Diabetes Mellitus e as manifestações bucais. Método: Os métodos aplicados incluíram estratégia de busca na literatura e critérios de inclusão e exclusão para a seleção dos artigos a serem adotados. As bases de dados incluídas foram PubMed, SciElo, BBO e LILACS, até setembro de 2011. Os artigos selecionados investigaram a associação ou não do Diabetes Mellitus com manifestações bucais, excluindo a doença periodontal. Resultado: De 1164 artigos encontrados, 30 deles estavam de acordo com os critérios de inclusão, podendo permanecer no estudo. As manifestações bucais foram associadas com Diabetes Mellitus em 20 estudos. Os desfechos relacionados foram candidíase, hipossalivação, líquen plano bucal, estomatite por dentadura e lesões linguais. Conclusão: Os pacientes portadores de Diabetes Mellitus estão mais predispostos a apresentar candidíase e hipossalivação, podendo agravar muito determinadas condições de saúde bucal.


Purpose: The aim of this study was to systematically review studies on the association between diabetes mellitus and oral manifestations. Method: The methods applied include a literature search strategy, inclusion and exclusion criteria for selecting the studies, characteristics of the studies were adopted. Data bases included PubMed, SciElo, BBO and LILACS until September 2011. Selected articles were related to clinical studies investigating whether or not there is an association between diabetes mellitus and oral manifestations, excluding periodontal disease. Result: Of the 1164 studies identified, 30 of them remained in the study due the inclusion criteria. The oral manifestations were associated with diabetes mellitus in 20 studies. The diseases candidiasis, hypossalivation, oral lichen planus, dentadure stomatitis and tongue lesions were related. Conclusion: Patients with diabetes mellitus are more susceptible to the candidiasis and hypossalivation, which in turn may exacerbate other oral health conditions.

RESUMO

O carcinoma de células escamosas (CCE) tem aumentado em pacientes jovens, principalmente em língua, correspondendo a 6% de todos os cânceres bucais. Sugere-se que essa neoplasia maligna seja uma doença distinta da que ocorre em pacientes mais velhos, com etiologia e progressão clínica particulares. Os receptores da família HER apresentam um importante papel na patogênese e progressão do CCE, sendo relevante a identificação de alterações no gene e na expressão das proteínas relacionadas ao CCE bucal...O CCE em pacientes jovens acomete mais a língua e não tem predileção pelo sexo masculino, estando o etilismo menos associado a esse grupo de pacientes em relação aos adultos. A imunopositividade observada para os anticorpos para HER-1 e p16 demonstra a participação dessas proteínas na carcinogênese dos CCE de língua e assoalha de boca. A presença de mutações nas regiões hot spots do gene HER-1 reforça a participação desse gene no desenvolvimento CCE de língua e assoalho em pacientes jovens. Futuras investigações são sugeridas nessa linha de pesquisa, para avaliar proteínas envolvidas na via de sinalização do HER-1, bem como nos casos p16 positivos, a presença do vírus HPV.

RESUMO

Hypoglossia is a rare entity which is reported sporadically. Hypoglossia has often occurred in an association with limb anomalies and, therefore, these cases have been grouped together as hypoglossia-hypodactylia within oromandibular limb hypogenesis syndromes. The literature on this condition is reviewed, and a case of hypoglossia has also been presented. An investigative protocol to aid in the treatment planning of a patient with hypoglossia has also been suggested.

RESUMO

Disturbance in the organogenesis of tongue might lead to some malformations like tongue tie, bifid tongue and hairy tongue. Severe degrees of these anomalies may cause speech impairment or periodontal defects. The present study was done on patients of the southern coastal belt of India during the past two years, on gross tongue anomalies. The results of the present study reveal that occurrence of tongue tie is 0.2 percent and bifid tongue is 0.3 percent in the southern coastal population. Since great majority of these oral anomalies have genetic basis the purpose of the present report is to highlight that these anomalies can exist without any familial background and also to suggest that environmental factor may play a role in the etiogenesis of these anomalies.


La alteración en la organogénesis de la lengua puede dar lugar a algunas malformaciones como anquiloglosia, lengua bífida y lengua vellosa. Grados severos de estas anomalías puede provocar un trastorno del habla o defectos periodontales. El presente estudio se realizó, durante los últimos dos años, en pacientes de la franja costera del Sur de la India con anomalías graves en la lengua. Los resultados del estudio revelaron que, en la población costera del sur, la incidencia de anquiloglosia era de 0,2 porciento y de lengua bífida de 0,3 por ciento. Dado que la gran mayoría de estas anomalías orales tienen base genética, el propósito del presente informe fue poner de relieve que estas anomalías pueden existir sin ningún tipo de antecedentes familiares y también sugerir que los factores ambientales podrían jugar un papel en el etiogenesis de estas anomalías.

Background: The main tributaries of the internal jugular vein join at cervical level through the venous thyrolinguofacial trunk. This trunk is classically described as formed by the union of the facial, superior thyroid and lingual veins. Aim: To evaluate variations in the formation of the thyrolinguofacial trunk in human cadavers. Material and Methods: Thirty hemi-necks were dissected in human cadavers of nine men and six women, with ages ranging between 29 and 50 years, and analyzed by direct observation. Results: In 16 hemi-necks (53.3 percent), the trunk was thyrolinguofacial; in seven (23.3 percent), it was linguofacial; in six (20 percent), it was thyrolingual and in one case (3.3 percent) it was thyrolinguo pharyngofacial. No thyrofacial trunk formation was found. There was a correlation between the diameter of the internal jugular vein and of the venous trunk formed by these veins. Conclusions: There are morphological changes in the formation patterns of facial, lingual and superior thyroid veins. This information is useful for surgical oncology, plastic surgery, head and neck surgery and radiology.


Los principales afluentes de la vena yugular interna se incorporan a nivel cervical por medio del tronco venoso tirolinguofacial. Clásicamente se ha descrito su formación por la unión de la vena facial, lingual y tiroidea superior. El objetivo de este estudio fue determinar las variaciones en la presentación del tronco tirolinguofacial en cadáveres humanos de la región del Maule, Chile. Se disecaron 30 hemicuellos pertenecientes a 15 cadáveres humanos de ambos sexos, con un rango de edad entre 29 y 50 años, y se analizaron mediante observación directa. Se observó la formación de tronco venoso en la totalidad de los casos, siendo 53,3 por ciento correspondiente al tronco de tipo tirolinguofacial, 23,3 por ciento linguofacial, 20 por ciento tirolingual y sólo un 3,3 por ciento el tronco tirolinguofaringofacial. No se encontró la formación del tronco venoso de tipo tirofacial. Además se encontró una correlación significativa entre el diámetro de la vena yugular interna y el tronco venoso conformado por estas venas. Por lo tanto, existen variaciones morfológicas en los patrones de conformación de las venas facial, lingual y tiroidea superior, siendo estos datos de importancia para áreas de cirugía oncológica, cirugía plástica, cirugía de cabeza y cuello y radiología.

Few studies have been conducted in the Saudi population on oral mucosal lesions. The purpose of this study was to evaluate the type and extent of oral lesions in a study among dental patients at a college of dentistry in Saudi Arabia. Over a 3-year period, 2552 dental outpatients were interviewed and investigated clinically for the presence of oral mucosal conditions. A thorough oral clinical examination was performed, including a radiographic examination. The diagnosis was confirmed histopathologically when necessary. Of 383 [15.0%] patients found to have oral mucosal lesions, females constituted 57.7% [n=221] and males 42.3% [n=162]. The age range of the patients was between 15 to 73 years with a mean age of 38.2 years. The most commonly affected age group was 31 to 40 years, which comprised 21.4% [n=82] of all affected individuals. The least affected age group were individuals older than 61 years. The most common lesion was Fordyce granules [3.8%; n=98], followed by leudoedema [3.4%; n=86] and traumatic lesions [ulcer, erosion] in 1.9% [n=48]. Tongue abnormalities were present in 4.0% [n=101] of all oral conditions observed, ranging from 1.4% [n=36] for fissured tongue to 0.1% [n=2] for bifid tongue. Other findings detected were torous platinus [1.3%; n=34], mandibular tori [0.1%; n=2] aphthous ulcer [0.4%; n=10], herpes simplex [0.3%; n=7], frictional hyperkeratosis [0.9%; n=23], melanosis [0.6%; n=14], lichen planus [0.3%;n=9] and nicotinic stomatitis [0.5%;n=13]. The findings of this study provide information on the types and prevalence of oral lesions among Saudi dental patients. This provides baseline data for future studies about the prevalence of oral lesions in the general population

A female child of 4 years with congenital absence of tongue has been described. This anomaly is usually associated with other congenital defects particularly limb and other cranio-facial defects. But in this case, the child enjoys the normal life, the unusual absence of tongue; she used to go to school play around with her friends and had a comparable I.Q.

Experience with frenuloplasty differs in different centers and there are many techniques in this regard. The purpose of this study was to investigate the severity of presenting symptomatology in patients with ankyloglossia and to assess the surgical results of patients undergoing frenuloplasty with split thickness skin graft. During a 4 year period from September 1998 through September 2002, 19 patients of ankyloglosia underwent frenuloplasty with a split thickness skin graft. All skin grafts were taken from arm. There were 11 males and 8 females. The average length of the lingual frenulum was 3.5 mm. Twelve children were over 4 years old and were primarily operated with this technique. In the 7 patients, Z plasty had been performed previously but failed and cicatrisation caused ankyloglosia. There were two minor postoperative complications, one hematoma, and other graft dehiscence with cicatrisation. The mobility of the tongue after one year was excellent. There were no complications in donor site. Frenuloplasty with split thickness skin graft is the best and easy procedure with good results for children over 4 years and those who fail primary closure

A língua é um orgão muito importante para a realização de funções como a mastigação, a deglutição, a fala, e participa de estímulos funcionais que atuam no crescimento e no desenvolvimento da face. É composto por músculos intrínsecos e extrínsecos que permitem sua movimentação tridimensional (altura, largura e comprimento). É necessário que se conheça a sua morfologia para se detectar anomalias como microglossia, macroglossia, anquiloglossia e língua geográfica. Por meio de exames morfológicos e funcionais, podemos verificar anormalidades em sua estrutura, obtendo-se um correto diagnóstico e tratamento adequado.

RESUMO

Proteus syndrome, which is a reference to the ancient Greek god Proteus, the Polymorphous, was called to the attention of pediatricians. Recognition of this syndrome has been difficult because of the variability of the syndrome's manifestations and because of its rarity. We describe a rare case of Proteus syndrome and we discuss its differential diagnosis. Our case presents with hemi facial hypertrophy, scrotal tongue, enamel hyperplasia as well as differential in the size of the dentition on both sides of the arch.

The present study included 132 cases with mental subnormality. The aim of the present work was to investigate the prevalence of the oral and dental abnormalities in patients with mental retardation and to delineate the most common patterns of presentation associated with oro-dental abnormalities. All patients were subjected to full history taking, thorough oro-dental examination and IQ assessment. Chromosomal studies and metabolic screening were done when needed. It was concluded that oro-dental abnormalities are more frequent in patients with severe or profound mental retardation. The oral region abnormalities represent a major problem in the oral health of patients with mental retardation. Medical dental services and care were recommended for the affected cases, since there is no national oral disease prevalence data for patients with mental retardation and developmental disability

Incontinentia pigmentii acromians (IPA) is a bizarre, irregularly shaped leukoderma of the trunk and extremities, characterized by marble cake-like whorls or streaks of hypopigmentation whose distribution can be unilateral or bilateral. Mode of inheritance is most consistent with an autosomal dominant type and in majority of the cases, one or more anomalies are present involving dermatological, dental and ocular disorders. A case of a female child, aged 2 1/2 yrs, referred from Pediatric neurology department, Medical College, Trivandrum with Incontinentia pigmentii acromians and seizure disorder is reported. Hamartomatous dental cusps and single maxillary central incisors have been associated with Incontinentia pigmentii acromians. A case of Incontinentia pigmentii acromians with hypoplastic teeth and tongue abnormalities is presented.

Because the tongue is superficially located and the intial manifestation of most diseases occurring there is mucosal change, lingual these lesions can be easily accessed and diagnosed without imaging analysis. Most congenital lesions of the tongue, however, can manifest as a submucosal bulge and be located in a deep portion of that organ such as its base; their true characteristics and extent may be recognized only on cross-sectional images such as those obtained by CT or MRI. In addition, because it is usually difficult to differentiate congenital lesions from other submucosal neoplasms on the basis of imaging findings alone, clinical history and physical examination should always be taken into consideration when interpretating CT and MR images of the tongue. Although the radiologic findings for congenital lesions are nonspecific, CT and MR imaging can play an important role in the diagnostic work-up of these unusual lesions. Delineation of the extent of the tumor, and recognition and understanding of the spectrum of imaging and the pathologic features of these lesions, often help narrow the differential diagnosis.
